To help facilitate your evaluation exam and reduce waiting time, print our forms and fill them out in advance of your visit. This is most helpful for people who wish to be seen as walk-in patients.

Medical Records
While Medical Records are not required to see one of our doctors, they are very helpful! We always prefer that you bring some record of previous evaluation or treatment for the condition you wish to have Medical Marijuana recommended for. Even if it’s just a page or two from a previous doctor, chiropractor, hospital, or clinic visit, a record of your care helps the doctor determine your present condition and how to best help you.
